Nine (9) Changes to OASIS Effective April 1, 2026

There are nine (9) changes to the new OASIS (E2) effective April 1, 2026. The changes include additions to the 3 - Resumption of Care (ROC), re-wording  of some questions and answers, and removal of O0350 Covid Vaccination from 6 & 7 Transfers and 9 Discharge OASIS.

(1) A1255. Transportation. In the past 12 months, has lack of reliable transportation kept you from medical appointments, meetings, work or from getting things needed for daily living?

  • REPLACES A1250
  • Changed to align with the Transportation item collected in other CMS programs.
  • Answered at SOC, ROC

(2) B0200. Hearing. Ability to hear (with hearing aid or hearing appliances if normally used)

0.    Adequate – no difficulty in normal conversation, social interaction, listening to TV
1.    Minimal difficulty – difficulty in some environments (e.g., when person speaks softly, or setting is noisy)
2.    Moderate difficulty – speaker has to increase volume and speak distinctly
3.    Highly impaired – absence of useful hearing

  • Added to ROC
  • Used for risk-adjustment of quality measures and must be available at the start of the quality episode (SOC/ROC). 

(3) B1000. Vision Ability to see in adequate light (with glasses or other visual appliances)

0.    Adequate – sees fine detail, such as regular print in newspapers/books
1.    Impaired – sees large print, but not regular print in newspapers/books
2.    Moderately impaired – limited vision; not able to see newspaper headlines but can identify objects
3.    Highly impaired – object identification in question, but eyes appear to follow objects
4.    Severely impaired – no vision or sees only light, colors, or shapes; eyes do not appear to follow objects

  • Added to ROC
  • Used for risk-adjustment of quality measures and must be available at the start of the quality episode (SOC/ROC).

(4) A0810. Sex

1.    Male
2.    Female

  • Replaces M0069 Gender

(5) O0350. Patient's COVID-19 vaccination is up to date

  • Removed from 6 & 7 Transfer and 9 Discharge OASIS

(6) A1110. Language

A. What is your preferred language? (15 options)
B. Do you need or want an interpreter to communicate with a doctor or health care staff? (yes, no)

  • Added to ROC
  • Used for risk-adjustment of quality measures and must be available at the start of the quality episode (SOC/ROC).

(7) J1900. Number of falls since SOC/ROC, whichever is more recent

B. Injury (except major): As described in the OASIS manual 
C. Major Injury: As described in the OASIS manual

  • Revised (B) and (C) response options to align with the J1900 item collected in other Medicare Post-Acute Care & Hospice settings

(8) M0102. Date of Physician-ordered Start of Care (Resumption of Care)

Month – Day – Year → Skip to A1255, Transportation, if date entered

  • Skip language changed from A1250 to A1255, to adjust for addition of modified Transportation item to the assessment

(9) M1000. From which of the following Inpatient Facilities was the patient discharged within the past 14 days?

NA Patient was not discharged from an inpatient facility → Skip to B0200, Hearing

  • Skip language changed for NA response, from B1300 to B0200, to adjust for the addition of B0200 Hearing and B1000 Vision to the ROC time point, which changed the sequence of items
